Custom Fields from Profile don't show up in the Record Survey Responses view

So, I'm trying to use the survey module in CiviCampaign to record the results of an architecture inspection of each
house in my neighborhood. Each house is represented as a contact.
I created the custom fields (for a 'ACC Survey' activity) and added them to a profile with it selected as being ok for CiviCampaign (although now if I create a new profile, I don't see the CiviCampaign option)
When I reserve and then try to record responses, the page shows up but none of my custom fields are in the list.
Is it because they're activity fields? I'm stuck.

Ok, I think I got it.. I needed to make the custom fields be associated with the Survey activity type. That wasn't clear to me in the docs.
