Name Field Disappeared When Creating New Contact?

One of our folks went to enter some new individuals into CiviCRM (running 4.3.3) and noticed that there was no longer a field for name shown. I checked and saw this was true also. Not sure what happened? I upgraded to 4.3.7 to see if this would resolve the problem - but it still exists. See attached screenshot. Any ideas?

Go to your Profiles and check the one called New Individuals

Are First and Last still there - have they been disabled or deleted?

So just to be sure - we are talking about the New Individual Profile that is a "Reserved Profile"?

do you have First and Last if you go to /civicrm/profile/create?gid=4&reset=1

how about at /civicrm/admin/uf/group?action=preview&id=4&field=0&context=group

Yes, the New Individual Profile that is a "Reserved Profile."
If I go to the first link I get a 404 error. The same with the second.
My URLs look more like this:
http://www.nameofdomain.org/wp-admin/admin.php?page=CiviCRM&q=civicrm/contact/add&reset=1&ct=Individual

ah sorry - don't know wordpress - suggest you compare the equivalent links with the wordpress demo site - hope that helps since my suggestions clearly aren't  ???

So, no guarantees, but I think the WordPress equivalent is:
http://www.nameofdomain.org/wp-admin/admin.php?page=CiviCRM&q=civicrm/profile/create?gid=4&reset=1
When I go there I get the following error:
"Sorry but we are not able to provide this at the moment.The requested Profile (gid=13) is disabled OR it is not configured to be used for 'Profile' listings in its Settings OR there is no Profile with that ID OR you do not have permission to access this profile. Please contact the site administrator if you need assistance."
What is weird is I requested gid=4 not gid=13?!

that error message does have the wrong gid and i could reproduce in 4.4. Can you please file an issue, we'll take a look at why its getting the wrong gid, which might help you with the original issue

Some additional information...
1. I think this may be a result of moving from Bluehost to DigitalOcean. I'm wondering if there is some component I don't have installed correctly that could cause this? Is there something special about the name fields? Do they use any special components? I went back in my backups and all the way back as far as I can go with DigitalOcean, I've had this problem...but I know it was working when I was on Bluehost (of course, this is my problem, not DigitalOcean's, they provide the VPS, I have to configure it right :P ).
2. I noticed that when I go to "Home" in CiviCRM neither "Activities" nor "CiviCRM News" displays anything anymore. I'm guessing this is related to the same issue? Is there some common functionality used between the two?
a. Also nothing appears when I go to "Organization Address and Contact Info"
b. From Email Address Options is also empty.
c. So is Message Templates.
d. So is Tags (Categories).
e. All other options under Administer-->Administratiion Console-->Configuration Checklist work. What is unique about these few screens?
3. I'm not sure if this is a known bug, or if this is only happening in my install - but it says "Powered by CiviCRM 4.3.3" down at the bottom, even though I've now upgraded to 4.3.7.

Okay, some more information. I've been looking at my apache error logs on the server and am seeing a lot of:

The next line is then an error reporting that the file could not be found like so:

This seems to indicate that CiviCRM is having some issues creating temporary files. I've checked the files directory and its subfolders and they all appear to be 0777, so there shouldn't be any file permissions issues.

It looks like I've resolved the issue by re-applying to files and all its sub-directories and files the 0777 permissions. I'd like to recommend some more friendly/web-based messaging regarding this error. There was no display from CiviCRM letting me know that it was having any problems with file permissions.

basically civi was trying to check through all the gids that i could potentially display and hence the error message
