Spent about 3 hours trying to get CiviCRM to pick up my case xml file. It turns out the problem was just that the directory structure is case sensitive - my directory was /sites/default/files/civicrm/custom_templates/CRM/Case/XML/configuration and it should have been sites/default/files/civicrm/custom_templates/CRM/Case/xml/configuration

I need to go and hit something now  ;D
