I found related thread about conditional field, but I think create new one is better.

A requirement like drupal cck module select or other ( http://drupal.org/project/select_or_other ), but need to implement in civicrm custom field or profile.

My question is:

1. Do anyone plan for this feature of CiviCRM custom field before?
2. If any, suggestion of database structure?
3. Current plan is implement a "relation" between different CiviCRM custom field (in the same custom field group). After define the relation, then secondary field will show only when user select a defined option in primary field. I'm not sure if this is a solution, or will conflict custom field design.

1. Yes but no one has designed or implemented it

2. I would say the user should decide the type of both fields, so u can have:

a. A select followed by a text box for certain options

b. A select followed by another select box wiht different options for different values in the first select

3. Your solution is a good start

Another option here would be to set it up as a textfield in Civi (so it will accept any input), then use Webform Integration to generate your form, which has a select/other widget.
