Wow, thank you very much Hershel!

Nowhere in the upgrade procedures did I see an instruction to delete any files from the drupal "files/" directory (just the civicrm module itself). I got rid of [SITE]/files/civicrm and was able to upgrade from 3.0.2 to 3.4.2 and then to 4.1.3 with absolutely no problems!

Perhaps this could make its way into the upgrade procedures doc somehow?

Yes it could. The docs are in a wiki so anyone can edit them. Would you like to edit that page and add a note about this? That would be great.
