If it is wordpress then it is:

http://www.mysite.org/wp-admin/admin.php?page=CiviCRM&q=civicrm/menu/rebuild

If it is Drupal then it is:

http://www.mysite.org/civicrm/menu/rebuild

This "menu rebuild" process is documented on step 15 and 16 of this page.

The "delete cache templates_c" is on step 9 of the same page.

Sorry to say but this didn't work for me on my wordpress site.  I think the same operation can be done in Administer>System Settings>Settings - Cleanup Caches and Update Paths where the menu has been reset successfully.  All the problems I have are still evident.  I am seriously considering reverting to civiCRM v 4.4.7 which was the last stable version I used.

It may be that there is a conflict with another Wordpress plugin.

[Modified] I have just disabled Jetpack and found that everything now seems to work in v4.5 as it should, subject to further testing.  Hoorah!.........

That's good to know.  I will add JetPack to my list of potentially troublesome plugins and try to investigate further at some point.

I have experienced JetPack errors on forms before, but never within the Civi admin interface.  Thanks, good to know.

FYI due to the large number of posts from people suffering major javascript failures due to incorrect url settings, we are switching core javascript to no longer rely on those settings on 4.5.1 (scheduled for this week) - this should make people's upgrade experiences smoother, although getting those settings correct is still a good idea.
Try asking your question on the new CiviCRM help site.

Having identified that it was JetPack that was causing my problems, I have investigated further and it seems to be the "Notifications" module that seems to be the culprit.  I found this by turning off each module in JetPack one by one.  Hope this helpful.

Have you tried to clean up cache?  Administer > System Settings >  Cleanup Caches and Update Paths > Cleanup Caches button

Yes, but it was only by turning off Jetpack>Notifications that the problems disappeared.
