your script looks to be running from - /home/username/test/sites/all/modules/civicrm/install/upgrade.php

But the include path looks to be set to - /home/username/public_html/sites/all/modules/civicrm/

Something looks wrong with your path settings.

That was the problem, a bonehead mistake I overlooked in the civi.setting file.  It upgraded successfully.  Now off to upgrade to 2.2.
