Your points are well taken.

In my situation, the client has chosen Mollom.  A custom module hook approach may be an alternative that helps some people in a similar situation to myself, where they do not have budget, time, or skill to create and maintain a patch to CiviCRM or Mollom.

We had the same problem.  After more poking around the forums I found that the solution involved
installing the Drupal module ReCaptcha  (I am on D6 and C4.2.7) (with Captcha). I  turned ReCaptcha on the user account creation page which uses a Civi Profile,  and turned mollum off on that page and now all is working!   Back down to 3 spammers a day which is more managable than the 20 a day last year!
