Including a Profile on Contribution Page - User Error or Bug?

I am attempting to set up a contribution/membership registration page.  As part of this, we need to gather some additional information (volunteer interests, skills, t-shirt size, etc).

I have set up a Profile to collect this information, tied to the custom Individual data fields.  However, when I get to the "Include Profiles" screen on the Contribution Page setup, it is my understanding that there is supposed to be a dropdown menu to select the correct Profile to include... instead what I see is attached below.  It looks like the screen is not rendering correctly for some reason because it does not look like the description in the user manual.

So, the question is this: am I doing something incorrectly in my procedure for including the Profile fields on the Contribution page or is this a bug in CiviCRM?  Once it is diagnosed, I would appreciate any help you could give in correcting the issue.

For future reference, a good place to start is to try the same steps on the relevant demo site.  https://civicrm.org/go/demo .

If you get the same results there it is probably a bug, if you don't then it could be user error or something wrong with your specific site.

I suspect it is that latter in your case, because even if you made some error is setting up your custom profile you should still have a drop down menu containing the reserved profiles that relate to individuals. 

Coleman (and others) would be able to look at your attachment and say "javascript crash" or  "Resource URLs configuration error" or "xxxx" , but I can't, so you will have to wait for someone with more knowledge than I to come along. 

In the meantime though, it may help them to diagnose the problem if you note down any other dropdown menus  or popup windows that work on the wordpress demo site, but don't work on your site.

Thanks for the response.  Attached is my test at the demo site... looks like it is some sort of issue with my particular install, so I'd be interested in advice on how to correct the error.  I've attached a screen capture from the demo site of what mine should look like.  This is the only instance of a broken component that I have found as I'm working through the site setup.

If you are using Jetpack, deactivate the Notifications module. That's what fixed it for me.

That solved it (deactivating the notifications module of Jetpack) - I'm not going to question why that works.  Thanks so much for your help!

I wrote the code for EBS payment gateway integration with drupal civicrm 4.5.5 version which can used for contribution.
