Backgroud: we are actually running a project wehere we are conducting a surveys with particular organisations. Surveys is based on question form which is usesd by reserecher during an interview.

What we need: we want to use civicrm profile with custom data fields to create that survey and use it to collect information during face to face interview (and at the same time put date to civic).

Problem 1.: we need special group of custom fields to collect information about appliction (just yes/no, name, adress) for some events in future steps of the project. In attachement I have enclosed how we wish to have it, but I don't have good idea how to prepare such table in civic. Or mayby we should to this table in other way, any ideas? Please help!

Problem 2.: at the end of our survey we need to enclose some legal statement about using collected data. We need this survey to be printed with these legal statement and signed by intervieved person. But, after we print filled survey, legal statment (which we put in profiles: pre form help) isn't printed.

in general for urgent problems, please consider hiring someone from http://civicrm.org/professional/

1. if you want to collect data the way your table is set up, your best bet would be to write some custom php code that does that. I do a fair amount of that in the SFSchool module (http://svn.civicrm.org/sfschool/trunk/)

2. if you need the legal statement and signature on anotehr page, you can use the redirect url feature to redirect the user to the legal statement page

3. r u printing the profile view? we do not show the pre/post help in view/search of profile
