Contribution form email not working

I set up a contribution page, and when I confirm the data, I get an error that says the email could not be sent, but if I go in the conguration for outbound email, and send a test message, it sends it fine.  How can I fix this?  Attached is a screenshot admin page.

Ok, here's something weird.  I filled out the form, but chose "On behalf of an organization", and entered a company name an primary email.  this contribution went through fine, no error???

any suggestions?
