Customizing CiviCRM -- How Easy, How to Integrate w/Future Releases

I'm new to CiviCRM and my client has some requirements which appear not to be supported out of the box. For example, they have a need to create event registrations in which multiple people can make donations of any size toward a single registrant's account. And there needs to be a dashboard where the registrant can see who has donated on his/her behalf and get his or her total of donations.

I'm thinking this will require modifying the CiviCRM code and I'm wondering about a couple of things. First, how well documented is the code (haven't looked yet) and how easy is it to modify?

Second, what happens with future releases? Is it easy (or possible) to externalize the custom code for special features like these so that the custom code can be easily integrated into the next version of CiviCRM?

Finally, what about reading and writing directly to the CiviCRM DB? Suppose I create a separate web application to handle the UI but want the data to be integrated with CiviCRM—is accessing the DB directly feasible?

Have you looked at what PCP = Personal Campaign pages has to offer in case you can twist that to your use case?

No, I was unaware of Personal Campaign Pages. At first glance, they look like they just might work for us.
