Multiple Participants Country Selection not updating state

We are experiencing an issue for participants 2 through 6 (we stop at 6).  The selection of a country doesn't fire the script to limit the selection of states.  This worked prior to upgrading from 3.12 to 3.13, but that does not mean that we didn't change something that may have affected this. 

Also,  we went live this morning at 9 AM and recorded about 800 registrations in our first hour with only 5 tech support calls - absolutely no server issues supporting this load.

I just noticed that it doesn't work for our custom fields on the first page, also.

OK, maybe it never worked for the custom fields.  My poor testers just don't remember.
