HTML label incorrect for custom radio field re-using a set of options

If two custom radio fields use the same set of options, the <label> tags for the second custom field point to the radio buttons of the first custom field.

To reproduce on the sandbox, create a custom alphanumeric radio field using the multiple choice option set "Most Important Issue". Now when editing the field, clicking a label in the second custom field changes the value for the first custom field. If you click the radio button itself, it works fine.

Another quirk, after creating the second radio custom field, the name of the multiple choice set is changed to the second custom field name.
