various CRM_Core_* errors when upgrading

I have upgraded from version 2.2.8 to 3.1.2 successfully.  The problem is I upgraded a db that is a month old instead of the current production version.  I have been able to roll back to 2.2.8, but when I try to upgrade again I get errors.  I have upgraded and downgraded over a dozen times over the past 24 hours and get one of the following errors when trying to run <url>/civicrm/upgrade?reset=1:

Fatal error: Class 'CRM_Core_Config_Defaults' not found in /home/admin/public_html/www/dev/sites/all/modules/civicrm/CRM/Core/Config/Variables.php on line 42

Fatal error: Class 'CRM_Utils_Cache' not found
Fatal error: Class 'CRM_Utils_Hook_Drupal'

Once I got this error:

Fatal error: Cannot redeclare class IDS_Filter in /home/admin/public_html/www/dev/sites/all/modules/civicrm/packages/IDS/Filter.php on line 52

I can see the classes being included, but why aren't they recognized?  I am out of ideas.

Update:

I wiped out civicrm and did a fresh install of verion 2.2.8 and copied over the production db.  Things seemed fine.  I then tried to upgrade to 3.1.2 and got some different errors.  Instead of classes not being found, I got class cannot be redeclared errors.

After trying to upgrade to 3.0.0, 3.1.1, 3.1.2, and 3.1.3, I am now getting this error:

Sorry. A non-recoverable error has occurred.

DB Error: a515ac9c2796ca0e23adbe92c68fc9fc

Database Error Code: Error on rename of './civicrm_dev/civicrm_group_organization' to './civicrm_dev/#sql2-f66-7e1' (errno: 152), 1025

BTW, I am NOT incrementally upgrading the versions.  When each version upgrade failed with some type error, I tried again with a different version.
