Do you mean the navigation menu on top? The simplest way is to use the standard function in Administer: Administer/Customize/Navigation Menu.

Alternative is to develop your own bit of code using the xmlMenu hook, more info: http://wiki.civicrm.org/confluence/display/CRMDOC/CiviCRM+hook+specification#CiviCRMhookspecification-hookcivicrmxmlMenu
