improving case creation form

I'd like to propose the case creation form fields be reshuffled. Personally, I think the case subject and type fields to be the most important, and yet they are toward the bottom of the form and easily overlooked. This is particularly true if there is a custom data set active on this form. I think the order should be:

Subject
Case Type
Case Status
Case Start Date
Medium
Details
Custom Data
Duration (some might argue this should be before custom data -- I suspect it's a little-used field, and thus should be at the bottom)

See attached screenshot and alternate template file for:

Hi,

They are important after the fact, but for the original sponsors they aren't determinable by the case managers until they've finished the interview, which happens in realtime. Case Type is equivalent to "what set of services does this client need", and that isn't known at the beginning. The format of the open case form is currently designed as a questionnaire.

Obviously the answer is to make everything extremely configurable. I'm just explaining the history behind the current setup, which is to match the sponsor's workflow.
