New to CiviCRM.
Checking if a household contact form already exists: 
- Would like to enter members of household in the same form
- Enter relationship to head of household like "spouse", "son", "daughter", "father1", "mother1", "father2", "mother2", etc.  Here I am talking about a large family where parents may be living with a family
- For each family member, would like to have fields for email and phone number

Would appreciate if such a form exists and if you would like to share it; OR if there are simple ways to create such a form.

You will need to do this as a custom form and collect the information. Please publish your code if you do so.
