Add Organization - missing fields

Hi,

On trying to add a new organization, three fields - organization name, legal name and nick name are missing (see attached screen dump).

This may have quite a simple solution, but I haven't been able to find anything too obvious in the administration settings.

Any assistance would be much appreciated.

Did you make any modifications to templates? If so, make sure they are correct and you didn't remove those fields by mistake.

Please also try to clean the template cache (search for specific steps on this forum or in documentation).

Check that those fields aren't hidden/display: none by id in you CSS document as well.

I ended up doing an update to 3.1.3, which reinstalled the templates and fixed the problem
