Register New Participant form has several copies of event custom data visible

Register New Participant form has several copies of  event custom data visible. The registration is kicked back if only one form is filled out, because the required fields need to be filled in.

How did the extra custom fields get into that form, and how do I remove them?

Did you add this custom fields to profile? If yes you can just removed it.
