Minor tweak for 'on behalf of' in templates

When you select 'existing organisation' the Label 'Organisation Name' disappears - I reckon it should stay if only so you aren't left wondering what just changed - and it is still the 'label' of the field.

Screenshots in case the above reads as gibberish

Also not sure if you care to tweak and put some space between the radion buttons and the field below - or is this just a Garland issue?
