Group selection on profile page

I have set up 6 groups which an user should select one of this.
Now how can create a voice for user profile?

You can include group field in a profile and then customize the profile template to add some javascript that will allow to select only one group.

Which kind of field I must select into profile for writing 5/6 different group?

You have to add Contacts -> Groups for groups to appear in the profile the way it has been done here for Summary Overlay profile:
http://drupal.demo.civicrm.org/civicrm/admin/uf/group/field?reset=1&action=update&id=26&gid=7
