Custom activities not working

I have create several custom activity types ie nurses visit, counseling etc and custom data fields to go with them ie dates and notes etc.
When I go to Other Activities on the Individuals card it brings up a form. There is the drop down to select the activity. When I select one of my new custom activities the screen refreshes to a blank space (no fields). Is this a bug or am I missing a step?

1. most likely you are getting a php error / php is dumping core. can u please check your error logs

2. can u try to reproduce this on our demo server?
