Missing state/providence

Not sure if other peole use for outside of America but we have found that there is no Middlesex in the State/Providence drop down for the United Kingdom. Is there away that I can make this change as a lot of our donors are from there.

Might want to check if Middlesex is a valid state/province entry based on the standard listing that we use (I think there's several old forum posts discussing the "right" source for this list for the UK). If it is valid and missing, perhaps one of you could submit a patch adding it to the default data.
