Questions about how email receipts are supposed to work

We've just had some custom work done on our contribution pages and things are now working differently. I'm not sure how they are supposed to work so I am hoping I can outline our system and someone can tell me whether this is core or custom.

  • Someone donates using their credit card. They are sent a CiviCRM receipt after the donation is processed by the payment processor, and they are returned to the Thank You page. They get the receipt configured in the 'Thank you and receipting' section of that donation page.
  • Their payment is recorded in CiviCRM as 'Pending'. An office administrator updates the contribution status from Pending to Completed.
    • If the office admin selects the contribution (from a search in 'Find Contributions' and chooses 'Update Pending Contribution Status' from the Actions dropdown, the person gets another receipt the same as the first. (not desirable - people think they are getting charged twice)
    • If the office admin clicks Edit on the contribution and chooses Completed as the status and leaves the Send Receipt box unchecked, the status is updated and the person does not get another receipt (desired).

My questions are:
1. Are they supposed to get a receipt when they contribute, or when the status is updated?
2. Assuming they're not supposed to get the one when we update the contribution status, how can it off? (Is it a configuration setting somewhere, or a template?)
