No controls appear in the Address field set on the contact/add form

Hi all,

I upgraded to 1.9 a few weeks ago, but I just noticed two problems, so I'm not sure when they might have appeared. Anyway, I could use some advice:
  • No controls appear in the Address field set on the contact/add form. I assumed I could fix this with admin/setting/preferences form, but
  • the expected check-boxes there are missing, too. I see the labels and help text, but no controls.

I use a common CiviCRM installation for about ten sites, and this only happens on two of them, so I assume its a problem in either the DB or civicrm.settings.php. I've done some cursory trouble-shooting, but decided I might get some better ideas posting here at the same time.

It is obvious to anyone how I've shot myself in the foot?  ;)

Addendum:

I've since repaired the Preferences problem by realizing that I didn't complete the DB update from 1.7 to 1.8 for all of my domains. That didn't solve the missing address controls, though. I could really use some advice on that one.

This sounds like a problem w/ some config settings stored in the DB or "possibly" a template caching issue. So...

1. Have you enabled debugging and included the directoryCleanup=1 param on a CiviCRM url?

2. Have you checked the values in Admin CiviCRM >> Global Settings >> Address Settings >> Address Editing section? Also, if you want to examine the DB state - the table and field which controls which fields are exposed in Address Editing is civicrm_preferences.address_options. Your value in this column should have the number 1 - 11 separated by the Ctrl A character.

Many thanks, Dave, for shaking my cobwebs loose. Your 2nd suggestion was the solution, and part of me knew it all along. I'd stared at that block for hours, not realizing that the fact all the address preferences were unchecked would prevent any controls from appearing in any address blocks! (Sometimes I hate having a one-track mind.)
