When sending Mailing, new duplicate contact is created

Every time our administrator 2 sends a mailing, a new duplicate Contact is created with just her email address.  (No First_name or Last_name which normally are mandatory in our system  ???
If she has sent, say, 3 mailings and then someone else sends a mailing she gets 4 copies!
This does not happen with other administrators.
(a) how or why is a new contact created?
(b) why are multiple duplicate copies sent to the same email address?

I would be grateful for any suggestion about what is causing this?

(I really like the CiviCRM 3.3.1. interface. Well done)

No idea, shouldn't happen indeed. Could you clean the db of the duplicate, login with her email and do the mailing yourself? I'd think that's a specific action that cause the pb, not a specific user.

Because that's different contacts. The "one only" rule is by contact, but if you have 10 contacts on your list that have the same email, it's going to send 10 emails

Ive logged in as her, created a mailing and it works perfectly.
I've just got to trace what she is doing.
