Email Block "invisible"

Can anyone tell me what is causing this?  I have imported data into Civicrm. If the contact had more than one email address, they show up.  However, if you try to add an additional email address by clicking on the ADD icon, you get what you see in the screen shot.  The field does not appear to be there. You can mouse over it and the system seems to see the field but it is as if it is truncated or something and does not show up. If I change themes it still happens and if I add a new record, I can add as many email addresses as I want during an ADD function, but I cannot go back and edit it and add an additional email address.

Thoughts?

Can you update your forum profile with CiviCRM version etc. Also try clearing template_c folder.
