"access denied" and no PDF receipt

A bit of a complicated environment:

CiviCRM 3.3.3
One patch applied to this:
"Site Name and Base Directory tangle and fight on a non-Multi-Site Drupal installation"

PayJunction is our CC gateway

Drupal 6.20
Secure Pages   6.x-1.8 with "civicrm*" in its rules
LoginToboggan   6.x-1.8

We are using CiviCRM to create Drupal ID's for our site granting the member access to premium content and other capabilities.

The one error we are seeing is that in the Drupal watchdog table there is an "access denied" as follows, and no PDF receipt is delivered.

Things which jump out at me...

"Location   http://domain.org/civicrm" which SecurePages is going to try to redirect that to https://

I have gone through all configurations I can think of verifying that NO WHERE is the www. version of the domain ever used. We did see some Civi forum posts talking about "access denied" being caused by mixing www. and non-www. URL's.

The ID is successfully created. The email to confirm the email addy is received, and by clicking the URL in that email the Drupal account drops out of the status "email not confirmed". So it seems there is this one part getting missed, otherwise the process is working well.

Suggestions please?

Update on this issue:

We disabled LoginToboggan's "Immediate login" as we are also using LoginToboggan's "Set password" capabilities.

Result: The Access Denied is gone, but so is the email "Welcome, no approval required" with the email verification URL.

So now new accounts are created, no errors what so ever in watchdog, and NO emails are sent... not the confirm email addy one, not the PDF receipt.

Suggestions please?

Last week, seems we had fat fingered the email addy on the test account. So, new week, anticipating we will get the "confirm your email" successfully.

I have verified in the email settings that Civi is able to send the test email. Our server is able to simply use the PHP mail() command.

Any suggestions as to why the receipt email comes up missing?

aaahhh!!!  ;D PJ leaves the CC transaction in a "Pending" state. Manually moving it along to the "Completed" state, and plop plop fizz fizz the receipt email is delivered.

So next off to chat with PJ as to how we can get transactions to end up in the "Completed" state.
