Responses to activities

Is there anyway to record a contacts response to an activity?

For example;
I create a new activity (direct mail) to a list of contacts.
The contact calls a unique number that was on the direct mail piece we sent out.
How can we record information we ask on that phone call to the contacts record with a reference to that activity?

Or is there a better way to setup the system to handle scenarios like this?
