Import formatting preventing the use of a few fields

When importing contacts using 4.0.alpha3 a number of critical fields are being covered up. This has been verified on Firefox 3.6 and Safari 5.0.3

The UI is covered up on the Choose Data Source and Upload CSV File and Import Options
