Problem with Groups in a Profile - 3.4

This is a real head-scratcher for me.  I'm in the process of upgrading from 3.2.1 to 3.4.  I have a membership form on the live site (running 3.2.1) which exposes the group field.  The member fills out the form and self-identifies as deaf, deaf/blind, hard of hearing or hearing.  It only displays the 4 group fields that I have set to public. 
This has worked fine for the last 2 years.

However, when I recently upgraded the site to 3.4 in test, I lost this functionality completely.  When I went into the member profile, I saw that the field (which I call STATUS) was no longer mapped to group(s) but now was pointing to Additional Address 1.  The odd thing was that the form still showed the 4 groups, however, when I remapped the field to group(s) and saved it, it now displayed all groups assigned to the contact.

In addition, I got the following error: 

Sorry. A non-recoverable error has occurred.
The requested Profile (gid=17) is disabled OR it is not configured to be used for 'Profile' listings in its Settings OR there is no Profile with that ID OR you do not have permission to access this profile. Please contact the site administrator if you need assistance.

Also, re: Groups.  I have an old smart group which I still cannot edit or delete for the past year now.  Everytime I try to delete it, I get a 500 server error.  These 2 issues may or may not be related but thought I would add just in case there is some correlation.

Any ideas?  I am really stumped.

Can you duplicate the error with the profile groups by creating a new profile with a similar configuration on our demo server or on a clean install?  That will indicate if it is a bug in 3.4 or some problem with your upgrade/install.

Will definitely do that but I mixed up my errors.  The error on test running 3.4 is this one:

Sorry. A non-recoverable error has occurred.
is not of the type Positive

and the error from 3.21 which is my live site is this:

Will let you know when I've tried this on demo.
