Undefined Index after version 4.0.1 upgrade

I am getting this error below when sending a receipt to the contact.  I can get the error to not occur by filling out the Honoree information on the contribution page.

Notice: Undefined index: honor_prefix_id in CRM_Contribute_Form_AdditionalInfo->emailReceipt() (line 322 of civi/sites/all/modules/civicrm/CRM/Contribute/Form/AdditionalInfo.php).

I am getting this error after an upgrade from Drupal 6 to 7 and civiCRM 3.3 to 4.01.

Not sure what 'Undefined Index' means (is this a DB problem), a faulty upgrade problem.

I am also seeing this error when I go into 'site preferences':

Notice: Undefined variable: drupal_wysiwyg in CRM_Admin_Form_Preferences_Display->buildQuickForm() (line 115 of civi/sites/all/modules/civicrm/CRM/Admin/Form/Preferences/Display.php).

I am also baffled by a comment I see on my contribution page which I do not see on the Demo.
The comment makes no sense since civiCampaign have been disabled and I do not have any Personal Campaign
pages on the system
 
     unlink from personal campaign page
Search for the Personal Campaign Page by the fund-raiser's last name or email address.
      
Name or nickname contributor wants to be displayed in the Honor Roll. Enter "Anonymous" for anonymous contributions.
   Personal message submitted by contributor for display in the Honor Roll.


Anyone seen this or care to advise?

i've fixed the undefined indexes for the next release

can you let us know what url you are seeing those messages in. They are not campaign related

The URL for the "Undefined Index" problem is: http:<url>/civicrm/contact/view/contribution?reset=1&action=add&cid=4771&context=contribution#
The URL for the "Undefined Varible" problem is: http://<url>/civicrm/admin/setting?reset=1
Another Undefined Index problem I'm receiving when updating the 'Domain Information' is on URL: http://<url>/civicrm/admin?reset=1
Also: Notice: Undefined index: phoneTypeId in CRM_Core_BAO_Block::create() (line 286 of <drupal root>/sites/all/modules/civicrm/CRM/Core/BAO/Block.php).
The bottom line is that I haven't probably seen all the 'Undefined Index' problems that may be generated, since these are just some random updates I've done.

What I need to know, since everything looks very bad for this installation at this point, is this civiCRM or me?  Should I try the upgrade again, start with a fresh install and then repopulate my data, or ignore these messages for the time being?

i would ignore the notices for now. they do not affect the working of civicrm (for the msot part). there is a drupal option in administer drupal to do so.

This just showed up under navigation on the Left hand menu:
Notice below that CiviCRM shows up twice, I did not see this after the install.
I disabled the second one for now, but it seems that these errors I'm seeing are cumulative.
I absolutely do not have two civicrm's installed and under administration/modules, civicrm appears only once.

i would disable civicrm and enable it again

this is mainly because there was an error when it was enabled the first time (or something like that)
