I'm using the on-behalf-of-org via profile functionality for the first time. great to see this added to core through an MIH project.

however, the insertion of the form into contrib pages seems pretty odd. it appears we run through and add the include file twice. that results in duplicate div id's -- and can be triplicate, because there's a hardcoded double use in the tpl file. i haven't spent enough time to understand the process flow for inserting it, but it looks like it needs some more work.
