new standalone custom form-joomla1.6 version

Hello,
can we create a new standalone custom form in civicrm to gather data and then do reports on it?
I do not want to club this form with profile/activity

I want to link such such custom form on admin top nav so that admin may enter data for this form. admin need to generate report on this form later

If yes then please please explain way to do so.
