Some users missing Mailings menu choice

First noticed with the "mailings" menu choice, but later determined that the issue is any menu change based on roles changes, does not seem to change the user's menu.

We've just converted from CiviCRM 1.9 to 4.0.5 and Drupal 7.   Users with Role for CiviMail have the menu choice.  However, when I now add the role additional users the Mailings menu choice does not appear.    For my own non-admin user id, I've changed my settings to match the user with the issue - only basic and civimail roles and I've still got the menu item mailings.

I've had the user change their password to a temporary one and logged in as them and duplicated the issue on my PC.

Any suggestions.  I thought about deleting the civimail role for me and adding back to see what happens, but I'm concerned I might not get it back.   Think I'll try a test account.

Created a brand new user "TestMenu" added basic and civimail roles - menus presented are as expected, including mailings

Deleted user with issue call "123".   Recreated user with same name "123"
Added basic and civimail roles - get menus different than expected

take away all roles from user "123"
log in - no civimail option showing, which is correct.
log out
add basic role
now have not only civicrm access on left menu but also civicrm with membership and contributions that don't have access to.

I think that menu changes are not happening for upgraded users - let's try for new user TestMenu created.

Adding Membership to new user TestMenu does not give Membership and Contributions menu choice.
Taking away CiviMail role does not take away Mailings menu choice.

Now the question is - is this an upgrade problem or a bug with drupal 7, civicrm 4.0.5

I hacked around a bit and the drupal user roles table is being updated properly, so maybe a static user menu table somewhere that is not being updated?

The issue seems to  be with menus for individual users, not the menu in general.

Or maybe a system cache someplace?  I've tried different browsers and clearing the browser cache without effect.
Tried http://<oursite>/civicrm/menu/rebuild&reset=1 - no effect
Tried above again with user with admin authority - now effect

For now, I've given the users the following temporary direct links to menu sections that are missing from their top menu:

New Mailing
http://<oursite>/index.php?q=civicrm/mailing/send&reset=1

Draft and Unscheduled Mailings
http://<oursite>/index.php?q=civicrm/mailing/browse/unscheduled&reset=1&scheduled=false

Scheduled and Sent Mailings
http://<oursite>/index.php?q=civicrm/mailing/browse/scheduled&reset=1&scheduled=true

Archived Mailings
http://<oursite>/index.php?q=civicrm/mailing/browse/archived&reset=1

Message Templates
http://<oursite>/index.php?q=civicrm/admin/messageTemplates&reset=1
