Option for using Saved Location missing

When using the Create Event wizard, the step that allows one to choose an existing saved location or type in a new location has an odd issue.   The radio buttons and select list to choose an existing location are missing. The address has to be typed in from scratch every time when creating a new event.

I have already cleared the CiviCRM cache under /sites/default/files/civicrm/templates_c 

Any ideas on what to investigate next?
