Single site only on multi-site install?

If I was to use CiviCRM on a Drupal multi-site install, but wanted to ensure that only one site could possibly have access to it, would it be advisable to install CiviCRM to that specific site instead of installing to 'sites/all' as normal?

Would this patch be the only thing I'd need to change for 4.0.0? http://issues.civicrm.org/jira/browse/CRM-7980

I suppose so. Can't say for sure without all the details of your sites and needs etc, but anyhow it sounds reasonable.
