case custom field option

the 4.1a1 upgrade script fails to insert the necessary _option_value record for case custom fields
(cg_extend_objects = _option_group)

as a result, case custom fields are not an available option in the custom field set option list.
support CiviCRM through 'make it happen' initiatives!
