Membership Purchase or Renewal Not Updating to Contact's Member Record

When a member purchases a member renewal or new membership, this membership is not being added to that contact's individual membership records.   It has to be entered manually. 

The user is paying online via a contribution page.   Settlement via Authorize (payment processor) is completing fine as well.    However, the Administrator is not received any corresponding transactional emails from CIVI.   The Bookkeeping Report and Contributions Dashboard both list the transaction and corresponding transaction IDs too.   When viewing the contact, the Memberships tab does not contain the corresponding renewal/new membership.

The issue happening on a fresh upgrade to 3.4.8, but existed for me in 3.4.4. (so I upgraded to attempt to resolve).  Everything was working at one point, however.   

I hate to have to totally rebuild, since I'm using a number of custom fields and don't think there is a way to export those fields (or the data)

Anybody have experience with this?

More Information:

One membership type behaves absolutely fine - all transactional emails and it being added appropriately to contact's membership tab.

Can you check your DB table (civicrm_membership and civicrm_membership_log) and see if there are any new records that are linked to this payment / transaction
