Yes I did test it on the demo site can could not recreate it.   

The only fields in the attached file area
First Name, Last Name, Email and Org employed by.

Here is the full error
Notice: Undefined index: 4_a_b in CRM_Import_Parser_Contact->checkRelatedContactFields() (line 1973 of /var/www/jenkins/live_jenkins-tfglive-5/sites/all/modules/contrib/civicrm/CRM/Import/Parser/Contact.php).

So if it is a setup/server issue on our end any thoughts on what it could be?

--Clean install Drupal 7 and CiviCRM 4.1.0

PHP Version 5.3.2-1ubuntu4.11
MySQL 5.1.41
Apache 2.0 Handler
Running mod_php

ditto on what stoob said. I often get these messages relating to the parser when doing imports but everything went through fine.
