Display problem on New Contribution form

I'm experiencing a weird issue with the New Contribution form.

Instead of providing the "credit this contribution to a personal campaign page" link that expands to provide Honor Roll fields, the form loads with the Honor Roll help text, no fields, and the "unlink from personal campaign page" link (which now does nothing).  See screen grab.

It seems like there is something peculiar to my install, but I can't think of what.  Any idea where I should go looking?

This is just a template issue in the case where no personal campaign pages exist.
