This occurs when you are looking at either the activity report, or the activities on an individual.

For an individual, this happens when you "view" the activity.
On the activity report, this happens when you click on the "activity type."

Also, as far as I can tell it is only happening on my user.
I have been unable to duplicate the error on any other activity of any user except for myself.

Notice: Undefined index: source_contact in CRM_Activity_Form_Activity->setDefaultValues() (line 476 of /home/user/public_html/sites/all/modules/civicrm/CRM/Activity/Form/Activity.php).

It does not happen on every activity. Only a few.
I don't know if that is relevant.
