"Contact us" profile is displaying a search result instead of the form (joomla)

I created a simple "contact us" form to display on our Contact page. I created a "Profile Create Form" menu item with the correct profile selected. Rather than displaying a form that people can fill out, however, it appears to be displaying a search result. ??? http://h2oforlife.server298.com/index.php/about-h2o-for-life/contact-us (you can find the page under About Us > Contact Us)

I've successfully created other forms, and as far as I can tell, all of my settings are the same.

Is there something obvious I might be doing wrong?
