CiviCRM membership on behalf of an organization - not working

Hello,
   We just went live with our partner membership page yesterday and are having problems.  I have the box clicked in the form "on behalf of an organization" so an organization will be created or recognized in our system and update or give a membership to the organization and attach an individual as the primary contact.  What has happened in "Test 1", the individual was created and given the membership and contribution, but the organization was not created.  In "Test 2" the same happened with the individual but the organization was created and attached though the membership still went to the individual.   ???

What version are you using?  Are you sure the membership type is for org and not individual?  I require on behalf of for certain membership types and thus set sign-up/renewal pages accordingly.  I separate my membership contribution pages based on membership type to ensure this is correct. 4.1.1, this is working correctly except when pre-populating custom data fields. Also, you have to enable the employer/employee relationship type correctly within the membership type. Hope that helps get you going in the right direction.
