Hello,
   We just went live with our partner membership page yesterday and are having problems.  I have the box clicked in the form "on behalf of an organization" so an organization will be created or recognized in our system and update or give a membership to the organization and attach an individual as the primary contact.  What has happened in "Test 1", the individual was created and given the membership and contribution, but the organization was not created.  In "Test 2" the same happened with the individual but the organization was created and attached though the membership still went to the individual.
