Submitting Profile Form Does Not Create a New Contact

Hi everyone,

I am using CiviCRM to create application forms and manage applicant information for our organization. Thus, I created a CiviCRM profile with standard and custom fields. I then took the HTML Form Snippet and pasted it into a webpage; however, when I submit the form, I get the following warning and the new contact information does not show up in the database (in other words, I can't find it using the CiviCRM Search feature).

     Note: this contact may be a duplicate of an existing record.

Any idea what may be causing this behavior? Let me know if I need to provide more information. Thank you, in advance, for your time and assistance.

Check the setting on the profile is case of a matching contact , civicrm profile handles in three ways
What to do upon duplicate match   
Issue warning and do not save
Update the matching contact
Allow duplicate contact to be created
In your case it issue warning and didn't save the profile ,so change the settings based on your organizational needs , From this point updating the matching contact seems solve the case.For more information refer this link:

Hello,

Thank you for the response and for the invaluable advice. I accessed the profile settings and changed the value of the 'What to do upon duplicate match' field. In fact, I tried both of the following options:

- Update the matching contact
- Allow duplicate contact to be created

The warning completely disappears when I use the 'Update the matching contact' option. However, neither of these appears to work in the sense that no email is sent to the administrator (as is the case with other profile forms) and I cannot find the contact using any of the CiviCRM database query features. It's difficult to debug because submission of the form does not generate any errors. Again, I appreciate any advice. Thank you, in advance, for your time and assistance.

Have you checked the settings for that particular profile?  In the Advanced Settings section, is there an email address in the "Notify when profile form is submitted" field that you can enter to ensure you are copied on any form submission?

Thank you for the reply. I did, in fact, enter email addresses in the "Notify when profile form is submitted?" field. This doesn't seem to have an effect.

It feels like the form is going nowhere upon submission. Any ideas what could be causing this type of behavior? Is there any other information I can provide to help diagnose the problem?
