Create User Record is not functioning in Joomla 2.5 and CiviCRM 4.1.1

Hello, I am really hoping to get some resolution on what seems to be a bug in the core.

When viewing a contact summary screen in Civi, and under actions, click Create User Record, I can try to create a username and password but on clicking "add" Civi then takes me to: url/administrator/?option=com_civicrm&task=civicrm/contact/view/useradd which is a completely blank screen. Turning on all joomla or civi debugging does not change this screen at all.

So basically, the ability to add a user record for access to my website is not operable right now.

My system is using Civi 4.1.1 and Joomla 2.5.4. any help would really be appreciated.

can u check your apache error logs and see if u can find an error reported there
