Problem with "Create user record"

Hi

We have a problem with "Create user record" for Joomla.
If you view a contact profile and go from actions->Create user record, it shows a form where it asks username and password. We can fill them in and click "Add". After that, it says user record created and everything seems fine. But it turns out that actually no user record was created. (Doing the same procedure again, you can choose the same username again).

Can anyone please help what the issue could be?

Joomla version 2.5.4 and civicrm version 4.1.1
