Confirmation emails work for free events but not for paid ones

We've had an issue for some months with event confirmation emails.  They get sent automatically if an event is free but they don't get sent at all if an event has a fee attached to it.  At first we thought it might be linked to a timing out issue we had but this has now been resolved and confirmation emails for paid events still don't get sent even though the correct boxes are ticked in Civievent.

I'd value any thoughts on this as the team are currently having to generate their own confirmation emails for each attendee and they're about to revolt!  Many thanks  :)
