CiviMail Job Stops 2/3 of the Way Through

I've got Civi 4.1.2 installed and working for over a month on a Drupal 7.14 set-up.

My client has been sending emails for about three weeks, two to three/week. The group he typically sends to has 12,000 emails. We are using CiviSMTP for the outgoing mail. No problems until yesterday.

Problem: About 2/3 way through, no new emails were sent out. The status of the email was listed as "Running."

I've got three questions:

1. What caused this?
2. What can prevent this from happening again.
3. What should I do if it happens again?

What I did: I was hoping to stop the email because it had clearly "hung." My hope then was to copy the email and send it again, excluding those who had already received it. I cancelled the email, which actually deleted it (with the records of who had opened it, clicks etc.). I ended up sending it to the whole list with an apology to those who got duplicates. This re-send went out flawlessly, as did another email that was sent out subsequently.

Ideas of what happened, how I can prevent it from happening again, and/or what I should do if it does happen again?

Did you ask CiviSMTP if something happened on their end?
